Forms needed by Graduate students

Welcome to the humorless world of departmental forms administration. We don't make the rules - well, sometimes we do - but we have to live by the systems that the University has put into place to validate and verify a grad student's course of study. We've added a few checks to the system to try to minimize the unpleasant surprises that come when students (and sometimes advisors or administrators) do not pay enough attention to how courses taken fit with the requirements of the department and the university. Feel free to consult with the Graduate Studies Coordinator about any of this.


First Semester of Enrollment

PLAN OF STUDY

Maryland Electronic Graduate SysteM

This electronic form needs to be completed in coordination with your Faculty Advisor by the middle of your first semester of enrollment. You are not expected to make final decisions about your coursework, but you should make best efforts at choosing courses which you believe will serve your educational objectives. The POS can be changed at any time. Your advisor and the Graduate Director sign off electronically on this form.


Internship
Internship Committee Report (PDF)

Bring this form to your Internship Proposal defense. It needs to be signed by each member of your committee. It is then returned to the Graduate Studies Coordinator to be put into your record.

Internship Evaluation (PDF)

This form must be completed by the person supervising your internship at the agency or company where the internship is happening. You should sign it and keep a copy before forwarding the original to your Faculty Advisor for review and signature. The Faculty Advisor then forwards the form to the Graduate Studies Coordinator.


Colloquim

The Annual Graduate Student Colloquium will be held in late April. You cannot graduate without fulfilling the requirement to present your work at the colloquium.

Paper titles and abstracts must be submitted to the Graduate Director by March 1st.

Deadlines for submission of these forms for the Spring 2008 semester are :

8 February– Application for Graduation (University deadline)
29 February – Approved Program form to Graduate Liaison (Department deadline)
28 April – Certification of Masters w/o Thesis (Deptartment deadline)

Note on Deadlines : The department's deadlines for submission of these forms is one week ahead of the official university deadline. We need that time to verify the information you have outlined on the forms. If you miss the department deadline we cannot guarantee that you will be able to graduate in the semester you have chosen. You also may be liable for extra charges associated with graduating in a later semester.

Application for Graduation
(online)

This is an online application.

Note : This application is due very early during the semester in which you plan to graduate.

Approved Program Form (PDF)

This form recapitulates your transcript. You must list all of the courses you have taken and the number of credits for each one. For courses which have not yet been completed, fill in all of the information except for the grade. The form must be signed by your advisor and then forwarded to the Graduate Studies Coordinator by close of business on the deadline date.

Certification of Masters without Thesis (PDF) This form usually includes the title of the presentation you have prepared for the annual colloquium and the date when the presentation was made. It must be signed by your Faculty Advisor and then forwarded to the Graduate Studies Coordinator by close of business on the deadline date.
