Listed below are some of the common forms that students may print and complete on their own. When completing the form, please read the directions and complete all of the applicable sections of the form before submitting. Be sure to note the proper procedure for the form. Permission to Enroll in Another Institution
If students want to take a course(s) away from the university, they must complete the Permission to Enroll in Another Institution form. Students must obtain appropriate approval(s) from the college and/or the department. Students are required to schedule an appointment or visit the BSOS Advising Center during walk-in hours to have their form reviewed and approved. If requesting permission to take a course for a major requirement, students must first seek permission from their department.
Official Audit Worksheet
Students must complete an Official Audit. An audit consists of the student reviewing Degree Navigator to ensure all requirements are being applied correctly, the department reviewing the student’s record, and then the college reviewing the student’s record.
Permission to Take a Graduate Course for Undergraduate Credit
Students who meet certain criteria are eligible to take a graduate level course(s). Student should print and complete the Permission to Take a Graduate Level Course for Undergraduate Credit form.
Late Diploma Application
If students did not complete the online diploma application by end of the schedule adjustment period of the semester in which they plan to graduate, they can print and complete the Late Diploma Application and submit to the BSOS Advising Center in 2148 Tydings Hall.
Appeal for Exception to Degree Completion Policy for Double Majors/Degrees
Students who want to add a second major or degree in BSOS and will exceed 10 semesters or 130 credits (whichever comes first, excluding AP/IB credits), must print and complete the Appeal for Exception to Degree Completion Policy for Double Majors and Double Degree form.
Benchmark Appeal Form
If students do not meet their benchmarks, they may appeal. This appeal must be submitted within 2 weeks of notification of the dismissal from the major.
Limited Enrollment Program (LEP) Appeal Form
1) Current LEP students may submit an appeal form for an exception to the gateway requirements, 2.0 GPA policy or the repeat policy. This form must be submitted to the Undergraduate Director of the LEP department.
2) Students wishing to gain admission to the major may submit an appeal form for an exception to: the repeat policy, repeat application policy, or to return to the major. This form must be submitted to the BSOS Advising Center in 2148 Tydings Hall.
