Services of the Hearing and Speech Clinic are available to anyone who seeks them, regardless of age. While referrals are frequently made by medical, dental and educational personnel for evaluation and/or treatment services, it is necessary for the client or parent/guardian to make the initial contact requesting services. The University of Maryland Hearing and Speech Clinic is a training and community service facility, therefore services are provided for minimal fees.

The client or parent/guardian must return the completed form by mail and have any relevant medical, psychological or educational forms forwarded to the clinic. An evaluation appointment will be scheduled after the requested information has been returned.
