Guidelines for Reconciliation Reports
All receipts should include the following information:
- Name of Study
- Subject's Name
- Subject's Social Security Number (not UID)
- Subject's Permanent Mailing Address
- Subject's Signature
- Source of Funding - FRsAccount
And should adhere to the following criteria:
- Form must be completed in ink
- Receipts must be dated after the date on the Petty Cash Check
And should include the following attachments:
- A copy of the Petty Cash Check should be attached to each reconciliation report
- A waiver for the subjects' SSNs must be received from the Controller's Office before the research begins
